Any Midd-West students who are interested in participating in athletics for the 2024-25 school year, can contact Mrs. Solomon through email at bsolomon@mwsd.cc for the correct paperwork (sign ups for fall sports were completed today in school and your children will have forms if they signed up). We will be offering physicals on May 28th and 29th at the high school during the school day. Anyone playing a fall, winter or spring sport can attend and get their physical for the year. If you already have a healthy roster account, you only need the memo below which explains the next steps. If you do not have a healthy roster account, please email Mrs. Solomon for the correct forms. If you want to get a physical at the high school on May 28th, all 8-11th grade students will need to sign up at the nurse's office for a time slot. All 7-8th graders will be brought to the high school on May 29th from 1-3:00 pm. These will be the only dates that physicals will be offered at the school district this year. You can still get a physical at your own doctor, it needs to be on the PIAA CIPPE section 6 form (attached). We will only accept forms through healthy roster, you can complete those digitally or upload a picture of your completed forms. PIAA requires all sections 1-6 completed before a student can practice.
